Ottawa Compassionate CareOfficial MI-DCH Application Forms
The official forms have been completed by the Michigan Department of Community Health. These forms are necessary to receive your Medical Marihuana Registry Identification Card. This is a packet of three forms actually. One form for your identification, one for your physician, and one for a caregiver should you need one.
